Seeing the Approval Minutes No. F.B. 14.079/2017 prepared by the General Directorate of Health Care and Public Health,
having regard to Article 46 of Government Decision no. 741/2016 on the approval of the Technical and Sanitary Norms on Funeral Services, Burial, Cremation, Transport, Decomposition and Reburial of Human Corpses, Cemeteries, Human Crematoria, as well as the professional criteria to be fulfilled by funeral service providers and the level of the fund Guarantee,
pursuant to Art. 7 para. (4) of the Government Decision no. 144/2010 on the organization and functioning of the Ministry of Health, with subsequent amendments and additions,
the Minister of Health issues the following order:
ART. ! 1
1. The model of the application for obtaining the operating health authorisation for the provision of funeral services and the model of the assessment report for the sanitary authorisation of funeral service providers, set out in Annex 1 to this Order, shall be approved.
(2) When issuing the sanitary operating permit, specify the activities carried out by the funeral service provider, in accordance with the NACE code.
(3) The sanitary operating permit on the basis of the evaluation report shall be issued according to the model laid down in Annex No. 8 to the Order of the Minister of Health No. 1.030/2009 on the approval of sanitary regulatory procedures for the projects of location, arrangement, construction and for the operation of objectives carrying out activities with a risk to the health of the population, with subsequent amendments and additions.

The model application for obtaining the opinion of the Directorate of Public Health for embalming, as set out in Annex 2 to this Order, is hereby approved.
ART. ! 3
The model of the tanatopractor's own declaration on the method and substances used, set out in Annex 3 to this Order, is hereby approved.
ART. ! 4
The model of the family's application for embalming, set out in Annex 4 to this Order, is approved.
ART. ! 5
The model of the application for obtaining the operating health authorisation for burial/burial activities (cemeteries) and the model of the evaluation report for the sanitary authorisation of burial/burial activities (cemeteries), as set out in Annex 5 to this Order, are approved.
ART. ! 6
The model of the application for obtaining the operating health authorisation for cremation activities of deceased persons (human crematoria) and the model of the evaluation report for the sanitary authorisation of cremation of deceased persons (human crematoria), as set out in Annex 6 to this Order, are hereby approved.
ART. ! 7
The County and Bucharest Public Health Departments will carry out the provisions of this order.
ART. ! 8
Annexes 1 to 6 shall form an integral part of this Order.
ART. ! 9
This order shall be published in the Official Gazette of Romania, Part I, and shall enter into force within 30 days of publication.
